Case Worker Job Description
Interfaith Sanctuary Homeless Shelter is seeking a part-time Case Worker. Schedule will be discussed to meet the needs of the agency and guests. Both daytime and evening hours will need to be covered.
Interfaith Sanctuary is looking for self-driven and enthusiastic employees to work along-side members of the homeless community who are in need of emergency shelter. While accomplishing daily work responsibilities, all employees are working to identify individual guest needs and the steps to successfully assist them in getting out of homelessness.
Job Description
• Works on a team with other Case Workers, assigned to a specific caseload of individual shelter guests to assist these individuals in ending their homelessness through needs assessment, goal setting, assistance in goal fulfillment, with regular follow up and accountability.
• Receives training in the following areas: o Shelter Policies and Procedures o Leading shifts
o SOAR
o HMIS, basic and case management software o Coordinated Entry
o Fair Housing
o Motivational Interviewing Techniques
o Other trainings as needed
• Utilizes motivational interviewing techniques to evaluate guest needs and goals
• Assist Guests in determining pathways out of homelessness
• Creates case management plan for each guest on their case load
• Meets with each guest on their caseload at least on a weekly basis
• Follows up with guests regarding case management plan progress
• Assists guests with Housing, Job, or other program application completion
• Ensures any assistance to guests is related to case management plan goals
• Tracks all interactions with guests using HMIS case management software
• Tracks all assistance given to guests using HMIS case management software
• Tracks all case notes, planning, and other documents related to case management using HMIS case
management software
• Attends weekly staffing meeting with Shelter Director/
• Actively communicates with the Shelter Director to discuss guest’s needs that fall outside of standard
operating procedures.
• Other duties as assigned by the Shelter Director.
Case Worker Qualifications

• Ability to work effectively as part of a team.
• Case management experience (not required) •

Compensation: Based on prior experience.
